Youth Island Club – Child Supervision Consent Form

Iftar & Taraweeh Program – Children’s Supervision (Ages K-5th Grade) Supervision Agreement:As part of the Youth Island Club's Iftar and Taraweeh program, we provide child supervision from 8:00 PM to 9:00 PM for children in Kindergarten through 5th grade. During this time, our volunteers will engage in educational and recreational activities.

By signing below, I, the parent/guardian of the child named above, agree to the following:

1. Consent for Supervision: I consent to my child being supervised by Youth Island Club volunteers during the specified hours (8:00 PM to 9:00 PM) and agree to ensure that my child is picked up on time.

2. Health and Safety: I understand that Youth Island Club volunteers will take reasonable precautions to ensure the safety of my child, but I acknowledge that accidents can happen. In the event of an emergency, I authorize the Youth Island Club to seek medical treatment for my child.

3. Behavioral Expectations: I understand that my child is expected to follow all guidelines and behavioral expectations while under supervision. In case of any misbehavior, I understand that my child may be asked to leave the program and I will be notified.

4. Liability Waiver: I, the undersigned, hereby release and hold harmless Youth Island Club and its volunteers, employees, and affiliates from any liability arising out of my child’s participation in the Iftar and Taraweeh supervision program.

5. Pick-Up Time: I will ensure that my child is picked up promptly at 9:00 PM.

Acknowledgment and Signature:I have read and understand the above consent form. I consent to the participation of my child in the Youth Island Club's supervision program during the Iftar and Taraweeh hours.
